If you have seen Dr. Aoueille within the last 7 years and you would like to request copies of your records, please click the ROI icon on the left to fill out our Release of Information Form (ROI).
Per Texas Medical Board's Guidelines, we charge the following fees: Paper Copies: First 1 - 20 pages = $25; 21+ pages = $0.50 per page thereafter Electronic Copies: 1 - 500 pages = $25; 501+ pages = $50 Records sent directly to your provider = No fee Once we receive your completed and signed ROI plus any applicable records fees, we have 15 days to respond under Texas Law.
